I work with children, youth, adults, couples, and families presenting mental health issues; behavioral, developmental and parenting difficulties, as well as people managing chronic pain and illness or undergoing critical life transitions such as immigration or divorce. The Carion Fenn Award for Courage and Bravery, is an award recognizing a person who has encountered a rare disease or disorder resulting in life changing circumstances. The award winner has not only persevered and helped themselves (and/or their family), but has also given back to others by showing bravery and courage in the face of challenge.

Award Nomination Form

The Carion Fenn Award for Courage and Bravery is awarded annually to an individual person as selected by the Carion Fenn Foundation Board of Directors. The award may be accompanied by a certificate, a presentation on behalf of the Foundation, and/or a prize that may include for e.g., a gift card for goods or services.

Eligibility Requirements:

  • Must have a rare disease or disorder, and given back to others by showing bravery and courage in the face of challenges. All eligble for the award. 

  • Board Members, volunteers, and employees of Carion Fenn Foundaiton  or it's sponsors/ partners not eligible for this award
  • A nomination form shall be submitted electronically to the Carion Fenn Foundation by August 31, 2016. 

  • Individuals may nominate themselves or be nominated by the public.

  • The award winner will be contacted by the Carion Fenn Foundation shortly after the Board of Directors selection of the winner.

Winner Must Attend the 2016 Rare Disease Expo Award Presentation physically or virtually.
